Please answer the following questions as completely as you can. The answers will help create a completely individual program for you pet, so we can best maximize his or her quality of life:

What do you feel are the most significant problems you are experiencing with your pet? What are the things keeping your pet from having the best possible quality of life?

What are your goals, both short and long term, for improving your pet’s quality of life? How do you expect this program to help your pet?

Has your pet suffered any injuries or accidents, even when very young, that may not be noted in his current medical record at East Lake?

What do you feed your pet? Please include the amount per day, as well as an idea of types and amounts of treats and food you share with your pet.

What medications or supplements is your pet currently taking? Please include any over-the-counter products as well as those prescribed by your veterinarian.

Do you feel your pet is an ideal weight? If not, how far from the ideal do you feel your pet is?

Has your pet ever been on a weight control program before? What did you do differently and was it successful?

Does your pet get any regular exercise? What kind and how much?

Does your pet know how to do any tricks or been through any obedience training?

We will probably develop an exercise program for your pet. Please tell us about your schedule and how much time per day per week you can commit to this program.

Describe any physical features of your home, yard, or neighborhood that may impact our program: Do you have stairs? Is the yard level? Can you walk to dog outside near your home?

Are you having any problems associated with your pet’s eliminations? (constipation, incontinence, etc)

Have you noticed ay behavioral changes (including things like pacing, confusion, increased barking)?
