East Lake Pet Orphanage Admittance Form

Filling out an application does not insure your animal of a place in our orphanage. We require that you continue to look into other alternatives while we assist you with the placement of your animal. Though we attempt to admit animals in the order that the applications are submitted, we must consider size, breed, sex, age, health, and disposition of each animal in an attempt to have a variety of animals available at any given time. This helps us to place animals in a more expedient manner. Due to the high number of animals in need of assistance, admission applications are automatically discarded 30 days after the submission date.

The cost of medical care and boarding for an adoptee can run from $100 to $400 or higher. To help defray these costs, I will contribute $ upon acceptance of this animal to the adoption program.

If this pet is accepted, I understand that I will no longer have any legal claim to it. I am aware that this animal may be humanely euthanized if it shows ANY aggressive tendencies, or if it has any serious health issues that diminish its quality of life. I will continue to try to find a home for this pet during its stay. During the time that the animal is under the care of the East Lake Pet Orphanage, I understand that they will perform any medical care they deem necessary. If this animal is a stray, I agree to put up signs and place and check newspaper ads in an attempt to locate its original owner. I understand that if I decide to return for the animal at any time, I will be required to go through the same adoption approval procedure as any potential adoptor. As the individual who released the animal, if I choose to adopt the same animal, I will be responsible for all medical and boarding costs incurred by this animal during its stay at East Lake Pet Orphanage, at full retail cost.

If East Lake Pet Orphanage is unable to accept this animal at this time:

Though we refer back to this information for future admissions, you should not consider this animal to be on a "waiting list." We will do everything we can to find a home for this animal, but unfortunately we are not able to help them all. Again, due to the high number of animals in need of assistance, admission applications are automatically discarded 30 days after the submission date.

We will be glad to provide a list of other humane organizations and will add this animal to our list of available animals in an attempt to match it with someone looking to adopt. I understand that a picture and brief description may be provided by me for the East Lake Pet Orphanage bulletin board located in our adoption room at no charge, and may also be placed on our web site for one month with a $20 donation to the orphanage (this donation is separate from the medical/boarding donation that we request at the time of admission) We also recommend the Internet as a source for humane organizations, especially breed specific ones, and for lost and found networks. We cannot return long-distance phone calls. Please provide local Dallas, Texas numbers if you are trying to place an animal.
